HISTORY
New Haven In-Home Care Inc. was founded by Eunice Bejar-Lee, RN in 2013. The organization is an outgrowth of a licensed and Medicare certified home health agency established in 2001. New Haven In-Home Care Inc. has been servicing the senior client population in San Francisco and San Mateo counties for the past 10 years before re-organizing, leading to becoming a separate home care organization (HCO). New Haven In-Home Care Inc. was incorporated on May 01, 2013.
New Haven In-Home Care Inc.’s referral sources primarily come from San Francisco and North San Mateo County. Many referrals are call-in inquiries from patient’s families who heard from the community about New Haven’s quality care and integrity.
For the past 10 years, our concentration was Live-In Caregivers supporting clients with hourly and 24 hour home care aides at home. Since 2013, New Haven In-Home Care Inc. added and developed partnerships with hospitals and skilled nursing facilities in San Francisco and San Mateo areas, becoming well-known in the community for success in Sitter Program, assisting and helping reduce hospital readmissions by keeping clients safe as part of hospital readmission program, thus aiding reduction of Medicare expenditures. In 2014, New Haven In-Home Care Inc. also added the Care Transition Caregiver Program, assisting hospital Emergency Department turnaround, ensuring patients discharged from hospitals and SNF are escorted and transitioned safely back to their homes after ER visits, reinforcing clients have basic needs of food, clean housing, assisting medication pick up from pharmacy and coordinate follow up care with home health visiting nurses and therapists.
